With this permission, you are able to set the biome of each realm area. For example, the Bay can have the "Basalt Delta" biome, and the Cliffs can have the "River" biome.
This has cosmetic effects, as the Nether biomes, for example, sometimes create additional particles, while other biomes change the color of the water. There is a lot to experiment with!
If you have a corresponding rank with the right to change your biome you can proceed as follows:
Type /realm in the chat.
Now click the settings wheel
Now click on Island Administration
At the right corner is a new symbol called "Set Biome"
Now select the area in the overview where you want to change the biome.
Now select the biome you want to set.
Alternatively, you can stand in the area and use the /realm setbiome <biome> command.
The choice of biomes depends on your rank, here is an overview:
When you have a higher rank, you also have access to the biomes of the previous ranks.
